We appreciate you scheduling your appointment well in advance, so your oral and facial cosmetic needs and treatments are never delayed or compromised. Please contact our office by calling or filling the appointment request form below. Our scheduling coordinator will connect with you soon and confirm your appointment. Please do not use the form for communicating private health information–you may securely do so while filling the Patient Registration Form here.  

If you have severe pain or think you need immediate attention, we will make every attempt to see you immediately. We always strive to stay on schedule to minimize your wait time.  

Dr. Sonneveld provides many types of oral surgery services, due to which the time allocated for a procedure may get extended. In addition to this, emergency cases may arise and cause delays. We appreciate your patience and understanding on these matters.
